  • Food Vendor Fees Per Event - $75
  • Must be insured
  • Appications will be reviewed and you will be contacted to confirm dates and make payment
  • VENDOR CHECKLIST Information (Check-in, Set-up, City of Berea requirements, Zero-Waste efforts)

Madison County Health Department
  • It is your responsibility to have all necessary permits and/or inspections.

Application submission signifies you have read and agree to these policies and agree to follow all items in the Vendor Checklist, including City of Berea requirements. ​​​

    I understand submitting this online application provides my electronic signature and I release FIRST FRIDAY BEREA / LEVITT AMP BEREA MUSIC SERIES, MOUNTAIN ASSOCIATION, CITY OF BEREA  and persons involved from all liability for any injury that may occur during this event. I hereby give my permission to FIRST FRIDAY BEREA, the MORTIMER & MIMI LEVITT FOUNDATION, and CITY OF BEREA to photograph, film or audio recording, in any manner, in all media, in perpetuity.
